6:45 PM – 7:15 AM
36 hours per week
Every 3rd weekend rotation

Sign-On Incentives:

$15,000 Sign-On Bonus paid over a 3-year period as a retention incentive.

Responsibilities:

Deliver safe, compassionate, and intentional nursing care.

	Serves as a liaison between GCGCH and the contractual EMS Ambulance Agency.
	Prioritizes and delivers routine patient care as delegated by the RN or other members of the interdisciplinary times.  This may include (but not limited to) collecting patient data; vital signs, intake/output, specimen collection, obtaining routine IV access, cardiac monitoring transporting patients.
	Assist with emergency resuscitation, set-up and assistance with bedside procedures/line insertions.
	Organize, direct and supervise work of other EMS members.
	Manage an annual budget, timesheets, financial records and required audits.  
Qualifications:
	Degree from an accredited 2 year or 4 year college or university in the field of EMS.  
	Must be an Iowa Certified Paramedic, National Registry preferred.
	A minimum 5 years of experience in the emergency medical service field, and a minimum of 2 years in EMS system planning and management with demonstrated leadership skills.  
	Must have current American Heart Association Healthcare Provider CPR or American Red Cross Professional Rescuer CPR.

At George C. Grape Community Hospital, our Social Services/Discharge Planning Coordinator serves to assist patients and their families navigate the process of illness and injury, from hospitalization to recovery.
 
GCGCH is currently looking for a dynamic team member who will help facilitate care prior to, during, and after a patient’s stay at GCGCH.  They will play an important role in connecting patients to needed services, coordinating payments and insurance reimbursements, and facilitate communication between patients and providers to ensure that the best patient care is provided.  They must also strive to increase the efficiency with which services are delivered and work to ensure that GCGCH is in compliance with policies and regulations that govern medical care.

Essential Functions 

•	Assesses patient/family within 24 hr. or next business day of referral.
•	Assessments to include identification of pre-hospital level of functioning and current patient/family utilization of community resources.
•	Assess and identify patients with limited support systems, level of functioning, and financial resources.
•	Assesses impact of illness of injury on social support system, level of functioning and financial situation, assesses psychosocial needs.
•	Utilizes standard form for assessments.
•	Aware of the medical plan of care and collaborates with care coordinator regarding post-acute patient clinical needs.
•	Communicates patient needs to the post-acute care provider.
•	Aware of community services and placement options available.
Discharge Planning
•	Communicates with patient/family options for post-acute care both private and community based and arranges same.
•	Formulates primary discharge arrangements and secondary plan.
•	Communicates with patient/family benefits/limitations for post-acute care.
•	Notifies patient, family, and care team of final discharge arrangements.
•	Referral as appropriate to Adult Protective Services (APS), Child Protective Services (CPS).
•	Communicates with patient/family options for care both private and community based.
•	Provides Medicare/Medicaid information to patient/family.

Essential Functions II

•	Track employee’s education requirements, Mandatory Reporter Training, and corporate compliance courses.
•	Coordinate in-house classes for required training and Continuing Education courses.

Essential Functions III

•	Participates in mandatory in-services and/or CE programs as mandated by policies and procedures/external agencies and as directed by management.

Qualifications

Job Overview

-The cardiac rehabilitation nurse will provide various exercises to patients in an effort to rehabilitate them from their procedure or complication and get them fully prepared to return to their daily lives.
